Add a Contact

  • Click on the “Add a Contact” button
  • Select the appropriate In-take Form

  • Select the appropriate Assigned To User
  • Identify the appropriate Sub-form(s) by their title

  • Fill in the Sub-form(s) with the relevant information
  • Click on the ‘Save and Exit ’ or ‘Save’ to finalize the contact
